Delivery shall be made through staggered deliveries based on the available warehouse storage capacity as determined by the Warehousing Unit. The Warehousing Unit shall issue a Delivery Instruction Form indicating the required quantity and the corresponding delivery period. The Supplier shall deliver the specified quantity within the delivery period indicated in the Delivery Instruction Form and shall not exceed the prescribed delivery period. Payment shall be processed upon complete delivery, inspection, and acceptance of the quantity specified in the corresponding Delivery Instruction Form.
The authorized representative should notify the Materials Management Section personnel at least three (3) days before
the delivery arrives for proper warehousing management.
For deliveries made through courier, the supplier shall properly coordinate and provide prior notice to the Materials Management Section (MMS), including confirmation that the shipment has been arranged and is scheduled for delivery.
In cases where the supplier’s authorized representative is not present during delivery, the Inspectorate Committee in coordination with the Materials Management Section personnel shall proceed with the inspection of the delivered items. Any damage, shortage, discrepancy, or non-conformity identified during inspection shall place the acceptance of the delivery on hold until the supplier’s authorized representative is present to witness and acknowledge the inspection findings.
Such inspection and handling of the delivery shall relieve the Procuring Entity, Inspectorate Committee, and Materials Management Section (MMS) personnel from any liability arising from the receipt, opening, and inspection of the package in the absence of the supplier’s authorized representative. The supplier shall remain fully responsible for replacing any damaged, defective, missing, or non-conforming items in accordance with the contract at no additional cost to the procuring entity.
The supplier shall ensure that all deliveries are properly handled and transported using its own personnel or authorized representatives. The procuring entity and its personnel shall not be responsible for any hauling, loading, unloading, or handling of the goods from the point of delivery. Any damages, losses, or discrepancies arising from improper handling or transportation shall be the sole responsibility of the supplier.
A copy of Purchase Order (PO) must be presented during delivery to ascertain which specific PO the item(s) corresponds to, especially in cases where bidders are awarded with multiple contracts.
7 Any product with an expiration date must have a minimum shelf life of eighteen (18) months from the date of delivery. If the product’s shelf life is less than 18 months, the supplier shall submit a guarantee letter, duly approved by the Medical Center Chief, confirming replacement of the item at least three (3) months before its expiration. Deliveries without the required guarantee letter will be outright rejected.
All items to be delivered must be securely packed in cardboard boxes, especially loose items, to protect the contents from damage and ensure cleanliness. Each box must be properly labeled with essential information such as the manufacturing date, expiration date, temperature requirements, and lot number. For loose items, the packaging must also include a clear label indicating “Loose Packaging” along with the corresponding quantity contained inside.
Strictly for items that require an SDS, a Safety Data Sheet (SDS) in the standard 16-section format must be submitted upon delivery. The SDS must have been issued or last revised within the past five (5) years; otherwise, it must be accompanied by a duly signed certification from the manufacturer or its authorized distributor/importer confirming that no changes have been made to the product information. Failure to submit the required SDS will automatically place the processing of the voucher for payment on hold until full compliance is achieved.

New notices are collected periodically, but this service does not routinely refresh already-known notices. Stored dates and status may miss deadline extensions, cancellations, or amendments.
