SUPPLY AND DELIVERY OF MEDICAL DEVICE FOR SCREENING POSSIBLE VISION PROBLEMS AND EYE CONDITIONS

INVITATION TO BID FOR THE SUPPLY AND DELIVERY OF MEDICAL DEVICE FOR SCREENING POSSIBLE VISION PROBLEMS AND EYE CONDITIONS 1. The City Government of San Jose del Monte, Bulacan, through the trust fund appropriation of 2026, intends to apply the sum of two million and seven hundred sixty thousand pesos (₱2,760,000.00), being the ABC to payments under the contract for: Supply and delivery of medical device for screening possible vision problems and eye conditions (BP-2026-10-0003TF). Bids received more than the ABC shall be automatically rejected at bid opening. 2. The City Government of San Jose del Monte, Bulacan, now invites bids for Supply and delivery of medical device for screening possible vision problems and eye conditions (BP-2026-10-0003TF). Delivery of Goods is required within ninety (90) calendar days from the date of receipt of Purchase Order. Bidders should have completed, within ten (10) years from the date of submission and receipt of bids, a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders (ITB). 3. Bidding will be conducted through competitive bidding procedures using a non-discretionary “pass/fail” criterion as specified in the IRR, otherwise known as the “New Government Procurement Act (NGPA)”. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA No. 5183. 4. Interested Bidders may obtain further information from the Procurement Management Section, 2/F New Government Center, Brgy. Dulong Bayan, City of San Jose del Monte, Bulacan, and inspect the Bidding Documents from Monday to Thursday, 7:00 AM to 6:00 PM. 5. A complete set of Bidding Documents may be acquired by interested Bidders from November 9, 2026, to November 23, 2026, from the given address and website(s) below and upon payment of the applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, in the amount of FIVE THOUSAND PESOS (₱5,000.00). The Procuring Entity shall allow the bidder to present its proof of payment for the fees in person. 6. The Committee will hold a Pre-Bid Conference on November 9, 2026, at 10:00AM at the Conference Room, Office of the City Administrator, 2nd Floor, New Government Center, Brgy. Dulong Bayan, City of San Jose del Monte, Bulacan, shall be open to prospective bidders. 7. The BAC Secretariat must duly receive bids through manual submission at the office address indicated below, on or before 9:00AM on November 23, 2026. Late bids shall not be accepted. 8. All Bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 16.1. 9. Bid opening shall be on November 23, 2026, at 10:30AM at the Conference Room, Office of the City Administrator, 2nd Floor, New Government Center, Brgy. Dulong Bayan, City of San Jose del Monte, Bulacan. Bids will be opened in the presence of the bidders’ representatives who will choose to attend the activity. 10.
