REPUBLIC OF THE PHILIPPINES PROVINCE OF PAMPANGA MUNICIPALITY OF PORAC INVITATION TO BID SUPPLY AND DELIVERY OF ONE (1) UNIT PATIENT TRANSPORT VEHICLE Name of Project Funding Source ABC SUPPLY AND DELIVERY OF ONE (1) UNIT PATIENT TRANSPORT VEHICLE Type of Vehicle - Utility Van 4x4 Engine Type - Common Rail Direct Inject Engine Displacement - 2.5L Transmission- Manual\ Fuel Type - Diesel PhilHealth Capitation P 1,595,000.00 1) The Municipality of Porac, through the General Fund intends to apply the sum of One Million Five Hundred Ninety Five Thousand Pesos (P 1,595,000.00) being the Approved Budget for the Contract (ABC) to payments under the contract for 2026-179 – Supply and Delivery of One (1) Unit Patient Transport Vehicle. Bids received in excess of the ABC shall be automatically rejected at bid opening. 2) The Municipality of Porac now invites bids for Supply and Delivery of One (1) Unit Patient Transport Vehicle. Delivery of Goods is required Sixty (60) Calendar Days. Bidders should have completed, at least ten (10) years from the date of submission and receipt of bids, contracts similar to the Project. Instructions to Bidders. 4) Bids will be evaluated and awarded to the MEARB (Most Economically Advantageous and Responsive Bid). 5) Bidding will be conducted through competitive bidding procedures using a non-discretionary “pass/fail” criterion as specified in the IRR of RA No. 12009. ________________________________________ [1] A brief description of the type(s) of Goods should be provided, including quantities, location of project, and other information necessary to enable potential Bidders to decide whether or not to respond to the invitation. i) Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA No. 5183. ii) Bidding is open to all interested Bidders, whether local or foreign, subject to the conditions for eligibility provided in the IRR. 6) Interested Bidders may obtain further information from the Municipality of Porac and inspect the Bidding Documents at the address given below from 8:00 a.m. to 5:00 p.m. 7) A complete set of Bidding Documents may be acquired by interested Bidders from the General Services and Procurement Office, Municipal Hall, Cangatba, Porac, Pampanga upon payment of the applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, of a non-refundable in the amount of P 5,000.00. 8) The Municipality of Porac will hold a Pre-Bid Conference on October 20, 2026 (Tuesday), 2:00 p.m. at Municipal Hall, Cangatba, Porac, Pampanga which shall be open to prospective Bidders. 9) Bids must be duly received by the Bids and Awards Committee (BAC) Secretariat through manual submission at the General Services and Procurement Office, Mun. Hall, Cangatba, Porac, Pampanga on or before November 3, 2026 (Tuesday), 1:30 p.m. Late bids shall not be accepted. 10) All Bids must be accompanied by a Bid Security in any of the acceptable forms and in the amount stated in ITB Clause 16.1. 11) Bid opening shall be on November 3, 2026 (Tuesday), 2:00 p.m. at Municipal Hall, Cangatba, Porac, Pampanga. Financial Eligibility Documents: ● Audited Financial Statements (AFS) stamped received by the BIR. ● Computation of Net Financial Contracting Capacity (NFCC) or Committed Line of Credit. c.) Technical Requirements: ● Bid Security (in the approved forms, amount, and validity period). ● Conformity with Technical Specifications (e.g., brochure, manufacturer's authorization, etc.). ● Schedule of Requirements (delivery timeline). ● Omnibus Sworn Statement. ▪ Financial Component (Second Envelope) The financial component focuses on the price quotation and ensures all costs are included. The arranged order generally includes: ● Financial Bid Form: Duly signed Bid Form stating the total price of the bid. ● Price Schedule(s): Duly signed Price Schedule(s) (either for goods offered from abroad or within the Philippines). 13) The Municipality of Porac reserves the right to reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to contract award in accordance with Section 70 of R.A.
