INVITATION TO BID FOR Supply and Delivery of Various Medical Equipment for Disaster Risk Reduction Management in Health (DRMM-H) under IB No. 2026-11-13 (69) 1) The Jose B. Lingad Memorial General Hospital (JBLMGH), through the Sub-Allotment from Department of Health-Central Office Quick Response Fund (QRF) intends to apply the sum of Twenty-Five Million Two Hundred Twenty-Six Thousand Pesos (PhP 25,226,000.00) being the Approved Budget for the Contract (ABC) to payments under the contract for Supply and Delivery of Various Medical Equipment for Disaster Risk Reduction Management in Health (DRMM-H) under IB No. 2026-11-13 (69). Bids received in excess of the ABC shall be automatically rejected at bid opening. 2) The JBLMGH now invites bids for the above Procurement Project using the Single Calculated Responsive Bidder (SCRB) and/or Lowest Calculated Responsive Bidder (LCRB). Delivery of the Goods is required as specified in Section VI. Bidders should have completed, within the period of ten (10) years from the date of submission and receipt of bids, contracts similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders. 3) Bidding will be conducted through competitive bidding procedures using a non-discretionary “pass/fail” criterion as specified in the IRR of RA No. 12009. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA No. 5183. 4) Interested Bidders may obtain further information from JBLMGH and inspect the Bidding Documents at the address given below from Monday to Friday, 8:00AM to 5:00PM. 5) A complete set of Bidding Documents may be acquired by interested Bidders on October 21, 2026 to November 13, 2026 from the address given below upon payment of the applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, depending on the total amount of bids using the JBLMGH’s ABC which will correspond to the standard rates indicated in the table below (mode of payment is Cash or Cashier’s / Manager’s Check payable under the name of the Procuring Entity): Approved Budget for the Contract Maximum Cost of Bidding Documents (in Philippine Peso) 500, 000 and below 500.00 More than 500, 000 up to 1 Million 1,000.00 More than 1 Million up to 5 Million 5,000.00 More than 5 Million up to 10 Million 10,000.00 More than 10 Million up to 50 Million 25,000.00 It may also be downloaded free of charge from the website of the Philippine Government Electronic Procurement System (PhilGEPS) and the website of the Procuring Entity, provided that Bidders shall pay the applicable fee for the Bidding Documents not later than the submission of their bids. 6) The JBLMGH will hold a Pre-Bid Conference on October 30, 2026 at 9:00AM and through video conferencing via Google Meet (the link will be provided in the confirmation message after submission of registration in Google Form: https://forms.gle/BEHrWNQtSWBGjmKr8), which shall be open to prospective Bidders. All participants must confirm their attendance by filling out the online registration form mentioned. In relation thereto, upon entering the Microsoft Teams room, the account must be renamed with the participant’s name and agency or company name. Only one (1) representative shall be allowed to attend the Pre-Bid Conference. 7) Bids must be duly received by the Bids and Awards Committee (BAC) Secretariat through manual submission at the BAC Conference Room, 4th Floor, Hospital Operations and Patient Support Service Building, JBLMGH, Dolores, City of San Fernando, Pampanga indicated below, on or before November 13, 2026 at 9:00AM. Late bids shall not be accepted. 8) All Bids must be accompanied by a Bid Security in any of the acceptable forms and in the amount stated in ITB Clause 16.1. 9) Bid opening shall be on November 13, 2026 at 9:00AM at BAC Conference Room, 4th Floor, Hospital Operations and Patient Support Service Building, JBLMGH, Dolores, City of San Fernando, Pampanga. Bids will be opened in the presence of the Bidders’ representatives who choose to attend the activity. 10) The JBLMGH reserves the right to reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to contract award in accordance with Section 70 of R.A.
